Diagnosis Codes 

Overview

 

The Diag. Code screen in the ClinicSource Setup section is where you add the diagnosis codes needed for invoices. After the codes are added you can select them from a list when you create invoices to submit to the insurance companies or when you create other types of documentation.

The tool-bar is where you Save , Delete , Print , select the discipline and enter the ICD-10 Implementation Date .



Add Diagnosis Code


The top part of the list is where you select the discipline you wish to update, add the diagnosis code, a description of the code, choose the version and associate it with a discipline.

Diagnosis Code List


The diagnosis code list displays all of the codes, descriptions and version added to the system.


Diagnosis codes are used by the medical community to group diseases, disorders and symptoms. The codes are also used to keep track of morbidity and mortality by the insurance companies. Codes are revised frequently as medical knowledge changes. There are several coding systems including:


  • International Statistical Classification of Diseases and Related Health Problems (Known as ICD) - Published by the World Health Organization (WHO) the ICD is in its 10th revision.
  • International Classification of Primary Care (ICPC) - The ICPC was first released in 1987 with a revision in 1998. In 2000, the ICPC was released electronically. Updates are labeled with a release date.
  • International Classification of Sleep Disorders (ICSD) - This coding systems is specific to sleep medicine.
  • North American Nursing Diagnosis Association International (NANDA)
  • Diagnostic and Statistical Manual of Mental Disorders  (DSM-IV) - This coding system is primarily used for psychiatric disorders.
  • Read Code - Used in the United Kingdom.
  • Systematized Nomenclature of Medicine (SNOMED)
